Transition planning supports students with disabilities and their families as they prepare for life after high school, focusing on education, employment, and independent living.

The Transition Coordinator collaborates with special education teachers to develop individualized transition plans (ITPs) for students with Individualized Education Programs (IEPs). These plans are tailored to each student's unique needs, strengths, goals, and interests.

Key responsibilities include:

Assessing Needs & Goals: Evaluating students' needs, aspirations, and abilities to design customized services and supports.

Career Exploration & Skill Development: Guiding students in exploring career options, building job-readiness skills, and practicing interview techniques.

Collaboration & Training: Partnering with Nebraska Western Region Transition Services to provide training and resources for teachers, parents, and staff on effective transition practices.

Progress Monitoring & Plan Adjustment: Regularly reviewing students& progress toward their transition goals and adjusting the ITP as needed to ensure ongoing success.
